Two (2) separate payments are required. One payment is for the application fee ($35) and one payment is for the Passport or Card fee.
To process your passport application,
You will need to bring
- Completed Passport Application – Filling out the DS-11 Form before your appointment will speed the process.
- State Issued I.D. – Out of State must provide secondary I.D. as well as Work, School, Military. Names must match. In case of Name Change, provide certified copy of proof.
- Proof of U. S. Citizenship – Original U.S. Birth Certificate with raised seal, Original Naturalization Certificate, etc.
- Important Documents for Name Change -If your name has changed, please bring the following ORIGINAL or CERTIFIED documents to your appointment:
– Divorce Decree (must show finalization of divorce and name change, if applicable)
– Marriage Certificate/License (must show new name)
– Court Order for Name Change (if applicable)Please bring paperwork for EVERY name change. Ensure all documents are certified or original to avoid delays. Copies will not be accepted.
- Minor – Both named parents on Birth Certificate must be present for minors (under 16) or Consent Form DS 3053 signed and notarized. Youth at the ages of 16 or 17 must have one parent present.
- Passport Photos – We can provide passport photos. Passport photo service is $15 per person.
